Fort Wellness Counseling at Restore and Revive Wellness Center

Getting Here

Please Note: We have moved to a new location at the Restore + Revive Wellness Center at 4927 Byers Ave #212, Fort Worth, Texas 76107.  

Fort Wellness Counseling is located at 4927 Byers Ave #212, Fort Worth, Texas 76107.  We are located within the Restore + Revive Wellness Center, which is just minutes away from downtown Fort Worth on the west side of town.  Our office spaces are located on the 2nd Floor.  We are currently occupying offices #208, #210, #211 and #212.

Inside The Building

When you walk into the office, the main waiting room is directly in front of you. You can check in at the front desk and let them know which provider you are there to see and what time your appointment is at.  The R+R team will let your provider know that you arrived and we will be with you shortly.  

Please make yourself comfortable in our waiting room.  Your counselor will come and meet you at the scheduled time or shortly thereafter.  Your intake paperwork will be at the front desk if it is your first appointment.

Appointment Reminders

We will contact you regarding your scheduled appointment a few different times.  Upon booking your appointment, you will receive a confirmation email and text message.  You will also receive automated reminders 48 hours prior to your appointment and 24 hours prior to your appointment.  You can opt out of these appointment reminders if you’d like. We put them in place to help you remember and for your convenience.  You will also receive an appointment reminder sent from your designated provider on the day of your appointment.  

Cancellation Policy

Our cancellation policy requires that you reschedule your appointment no more than 48 hours prior to your scheduled appointment. If you require rescheduling within 24 hours of your scheduled appointment, you will be responsible for paying the cancellation fee which is the full-service price of the counseling service that you had been scheduled for.  Please feel free to reach out if you ever have any questions or concerns.  You can learn more about what to expect at your first counseling session.  You can also check out our blog to help familiarize yourself with our practice and some of the services we provide.
